Overview

Care Coordinator

Full-Time; FTE 1.0

Monday-Friday; 8:00am-4:30pm

Moline, IL

The Care Coordinator is supervised by the Robert Young Manager of Community Based Services. The Care Coordinator will provide mental health assessment, intervention, and consultation to individuals identified as having psychiatric disorders; plan treatment modalities, coordinate treatment plans and follow clients through the treatment process; may supervise clinical interns assigned to department.


Responsibilities
  • Assures that all clients receive clinically sound triage/referral and ongoing care management services for behavioral health needs.
  • Performs diagnostic assessments and collects diagnostic information to determine an individualized treatment plan for the client.
  • Plan treatment modalities, coordinate treatment plans and follow clients through the treatment process
  • Requests consultation for clients that includes providers, social workers and nursing staff when necessary.
  • Delivers high quality care appropriate to the age and educational level of the client.

Qualifications

Education:

  • Requires bachelor's degree with at least 30 semester hours or equivalent quarter hours in a human services field, including but not limited to sociology, social work, counseling, psychology, or human services.

Experience:

  • Experience working with persons with severe and persistent mental illness.
  • Two years supervised experience in mental health or human services is preferred.
