The Care Manager III (CMII) provides intensive case management services for a caseload of up to 20 high risk Health Plan members. The CM III is responsible for the assessment of clients with multiple medical and psychosocial needs and plans, monitors services and interventions ensuring provision of quality care. The CM III also provides mentorship and training for all new case management staff and interns.
