1st Shift: 7:00 AM - 7:00 PM | Monday - Friday Pay: Paid per case completed per month | Depending on experience Requirements: To be successful in this role, you must have a minimum of two years of experience with case assessments, hold a valid Social Worker (SW), Registered Nurse (RN), or Physician Assistant (PA) license, or possess a bachelor's degree in human sciences. It is also necessary to provide your diploma or transcripts, valid driver's license, and car insurance. We have tons of perks!
Job Duties: As a Case Manager, you will accomplish clients' care by assessing their treatment needs and developing, monitoring, and evaluating individualized treatment plans and progress. Your role will involve coordinating services, arranging necessary resources such as transportation and escort, and closely tracking cases by verifying clients' attendance and assessing their treatment responses. Additionally, you will advocate for needed services and entitlements, obtain additional resources, intervene in crises, and provide personal support to clients.
