Job Summary

The Respiratory Disease Case Manager (CM) provides comprehensive clinical support and education to patients diagnosed with a respiratory disease. Clinical support includes: performance of cardiopulmonary diagnostic procedures (i.e. PFT's,ETCO2 monitoring, spirometry, collection of sleep study NOC Ox data, care plan management, results reporting to physician, patient assessment, application of RT driven protocols, home downloads of mechanical ventilator usage parameters, correct fitting and troubleshooting mask interfaces, documentation of care. This position also provides education and training to colleagues, patient, family and caregiver as applicable including: normal lung anatomy & physiology, physiologic changes caused by respiratory disease, medication classifications, medication reconciliation: what, when where, how and why they take it, recognizing exacerbation or decline, pulmonary rehabilitation and exercise, smoking cessation, oxygen use, mechanical ventilation, trouble shooting mechanical airway clearance strategies

Minimum Qualifications - For full consideration, applicants are encouraged to upload license and/or certification if required of the position

  • Current and valid California Respiratory Care Practitioner License.
  • Credentialed and valid as a Registered Respiratory Therapist (RRT).
  • Current Basic Life Support (BLS) CPR Certification.
  • Bachelor's degree completed, or in progress.
  • Three (3) years as a Respiratory Care Practitioner in an acute care hospital.
  • Expert knowledge of clinical respiratory care assessment and respiratory equipment utilized for ventilation, hyperinflation, and mucus clearance.
  • Performs basic and advanced cardiopulmonary diagnostic procedures and calculations.
  • Collects and interprets data to facilitate objective patient assessment and effective care planning.
  • Assists other health care professionals in the interpretation and clinical application of results.
  • Knowledgeable of and compliant with all applicable infection control and health and safety policies and procedures as implemented by the department and the UC Davis Health.
  • Professional communication skills to interact with therapists, nursing, physicians and department leadership.
  • Strong understanding and ability to use the core Microsoft Office applications: Word, Excel, and Outlook.
  • Knowledge of Electronic Medical Records (EMR) System.
  • Effective written communication skills.
  • Knowledge of and ability to teach and mentor staff.
  • Working knowledge of basic management regarding chain of command, and conflict resolution.
  • Time management and organizational skills.
  • Continuing maintenance and development of clinical skills and technical competency in Adult/Pediatric respiratory care as well as Emergency and Trauma respiratory care, applications, procedures and equipment.
  • Knowledge of regulatory requirements (JCAHO, Title 22,HCFA,CMS).

Preferred Qualifications

  • Certification as a Tobacco Treatment Specialist.
  • Certified Case Manager (CCM) obtained through Commission for Case Manager Certification (CCMC).
  • Master's Degree preferred.
  • Prior demonstrated experience planning and conducting in-servicing education and training.
  • Current working knowledge of up-to-date Global Obstructive Lung Disease (GOLD) guidelines, Global Initiative for Asthma (GINA), care standards for Duchen's Muscular Distrophy (DMD), Spinal Muscular Atrophy (SMA), and Amyotrophic Lateral Sclerosis (ALS)
  • Excellent interpersonal and communication skills

Department Overview

Clinical support and education and training is provided with the goal of increased quality of life, better outcomes and decreased hospital admissions for these patient populations. Case Managers utilize written education materials along with educational aides such as handouts, lung models and videos as deemed appropriate. When applicable they provide referrals for pulmonary rehabilitation and pulmonary function testing. They may assist in obtaining durable medical equipment or prior authorizations from insurance companies. They work directly with the durable medical equipment companies to ensure home care and equipment is appropriate and the follow up education occurs. They act as a liaison to physicians to ensure that appropriate standards of care are observed (GOLD Guidelines, GINA, DMD, SMA, ALS), and that adequate education and follow up occur. During the course of a hospital admission or clinic visit, the CM may administer pharmacological and therapeutic agents related to respiratory care, and may transcribe and implement written and verbal orders of a physician pertaining to the practice of respiratory care. CMs provide follow-up care to answer questions and ensure appointments with primary care physicians. CMs keep meticulous records to ensure patient continuity of care, performance improvement and research databases are intact. CMs offer education to coworkers and the medical staff. CMs workload and schedule may vary depending on clinic needs.
