Scope of Position/Position Summary The Care Management Lead works under the direction of the Care Management Leadership Team leading team in providing services directed to achievement of positive patient outcomes, maximizing the patients involvement in his/her own plan of care, improving service levels, reducing length of stay and improvement cost management. In addition to basic standards of Acute Case Management, the Lead is expected to role model and exceed staff Standards of Performance. The Care Management Lead assists department leadership with planning, organization, implementation, and evaluation of care management services. They provide clinical expertise support to staff, serves as a resource with complex transition planning, and mentors staff in the practice of care management and provides direct care management services, as needed. Serves as a resource to staff in the use of evidence-based criteria. The Care Management Lead maintains an awareness of state and national health care trends and regulations including regulatory agencies and the Centers for Medicare/Medicaid Services and monitors care management practice to ensure compliance. Minimum Qualifications For Hire: Bachelors degree required; Masters preferred. RN with five years recent hospital clinical case management experience or Master of Social Work from an accredited Graduate School of Social Work, State of Ohio Social Work licensure. LISW required. At least 3-5 years of experience as a Social Worker. Must be able to work with various computer applications. |
