JOB PURPOSE:

The Case Manager/Intake Coordinator is a dual role position where 50% of the time is dedicated to the Intake role and 50% is dedicated to Case Management. This position will work with a multi-disciplinary team for case assignments, consultations, and care coordination. This position facilitates direct referrals for housing, employment, healthcare, legal, educational, peer-support and all other essential community-based needs for clinic clients, will conduct phone and in-person referral screeners using an electronic health record system to initiate services for eligible clients. This position manages referral partnerships by continuous engagement post referral, ensuring the provision of quality care. Assists the clinical team in understanding resources and needs within the clinic community. May also provide direct client services using evidence-based targeted treatment as authorized by the Lead Clinician/Clinic Director.

ESSENTIAL JOB RESPONSIBILITIES:

  • Conduct initial referral screeners and intakes (as needed) with Veterans, Active Duty service members and /or family members. Coordinate case assignment with clinic multi-disciplinary team. Document detailed and confidential intake information in the electronic health records (EHR) system; maintain a comprehensive client referral tracking system. Prepare reports as requested by Clinic Director/Lead Clinician.
  • Interviews and provides comprehensive case management to veterans, active duty service members, and their families who are seeking mental health services. Provides appropriate referrals as necessary.
  • Prepares and maintains confidential case records. Consults with service providers and community partners on resource-related issues. * Ensures confidentiality and controls access to sensitive information.
  • Collaborates with clinical team to perform risk management and crisis intervention services as necessary.
  • Assists and supports the Intake Coordinator and Case Manager positions in their duties.
  • Provide support to Office Manager/Clinic Director for managing insurance billing requirements when needed.
  • Develops network of trusted contacts within organizations in which to refer and coordinate care.
  • Participates in multidisciplinary team meetings.
  • Follows care follow-up procedures to ensure veterans, active duty service members, and/or their family received the services they were referred to and do not have any follow up needs.
  • Assists veterans, active duty service members, and their family members navigate service providers across multiple systems of care.
  • Provides services via telehealth platform, as required.
  • Other duties as assigned


ESSENTIAL QUALIFICATIONS:

EDUCATION: Master's Degree in Social Services or Counseling required. Active license in the state of Texas at provisional level e.g., LPC-Associate, LMFT-Associate, or LMSW strongly preferred. Maintain license and/or certifications as required by state regulations. Stay current in the field of mental health, counseling, social work, case management and other related social services.

EXPERIENCE: Case management, intake, counseling experience preferred. 2+ years in a customer service focused environment. Experience working with military or veteran population is preferred. Knowledge of/experience working with local social service, public health, and VSO organizations and resources. Experience required in assessing immediate risk to self or others for the population served. Bilingual candidates are highly preferred.

ATTENDANCE: Must maintain regular and acceptable attendance at such level as is determined in the employer's sole discretion

LICENSES: Driver's License with clear record required.

VEHICLE: Must have daily use of a vehicle without prior notice.

OTHER: Must be available and willing to travel to various locations and with such frequency as the business need dictates. Must be available and willing to work nights, weekends and holidays as required to meet business needs. Must not pose a direct threat or significant risk of substantial harm to the safety or health of himself/herself or others.
