Duration: 6 Months

Shift Timing (Day/ Evening/ Night): Day

Job Description:

  • Assists clients in accessing services available through public welfare and community agencies, needed for them to maintain self-sufficiency and housing.
  • Develops resource files on community facilities such as social agencies, clinics, recreational facilities, or nursing homes.
  • Completes home visits for updates on the client's current situation and creates plans for needed services.
  • Keeps adequate records in the software for the assigned program.
  • Prepares brief reports and recommendations for supervisor.
  • Assists isolated individuals in finding satisfying social contacts and preventing homelessness.
  • Coordinate services by managing continuum of care initiatives.
  • Knowledge of social and economic conditions in the community and of welfare practices.
  • Job responsibilities in Brief
  • Primary case manager for a caseload. Clients include individuals who were formerly homeless and have a disability (meeting HUD's definition of chronic homelessness).
  • Duties include telephone, in-person, and home visit client contacts.
  • Maintain client records in compliance with HUD standards. Income verification, identification of other service needs.
  • Provide information, referral, and client education.
  • Knowledge, skills, education, and/or experience:
  • Ability to establish relationships of mutual trust and respect with clients, especially with disadvantaged people of varying ethnic and social background.
  • Ability to follow oral and written instructions and work independently and make practical application of skills and knowledge acquired through training, experience, and education.
  • Ability to Interview clients, conduct assessment, identify resources, assist clients through housing readiness to housing stability.
  • Ability to enter case notes, complete HMIS data entry. Conduct telephone, face to face and in-home consultations with clients, and collateral contacts on behalf of the clients.


Mandatory skills/certification(s) Requirement:

  • Excellent written and oral communication skills, seek supervision and communicate with supervisor regularly.
  • Bi-lingual (English Spanish).
