Position Overview

As a Mental Health LPN, you will play a crucial role in conducting focused assessments of patients' health status and collaborating with providers and Registered Nurses to coordinate psychiatric services. Your expertise will help ensure that our patients receive the highest quality of care in a clinic setting.

Key Responsibilities

  • Patient-Centered Care: Provide care in a non-judgmental and sensitive manner, respecting each patient's autonomy, dignity, and rights.
  • Clinical Interventions: Perform interventions as ordered by physicians, including lab draws, EKGs, and other necessary testing, while accurately documenting results.
  • Effective Communication: Screen calls from patients and families, addressing needs or transferring calls to the appropriate clinic nursing support team member.
  • Data Management: Track and communicate pertinent medical and psychiatric information through verbal and written documentation in the Electronic Health Record (EHR).
  • Care Management: Utilize psychosocial, physical, and behavioral data to provide individualized care management for patients, both in person and over the phone.
  • Medication Management: Complete medication refills accurately, administer long-acting injections, and educate patients and families about medications.
  • Collaboration: Work closely with interdisciplinary team members, seeking timely advice and providing feedback to ensure safe patient care.

Qualifications

What We're Looking For

  • Educational Background: Graduate of an accredited school of nursing with current licensure as an LPN in Minnesota.
  • Experience: One year of psychiatric/mental health nursing experience within the past two years is preferred.
  • Critical Thinking: Strong problem-solving skills and the ability to make safe judgment calls in stressful situations.
  • Communication Skills: Ability to communicate professionally and effectively in both verbal and written formats, modeling a positive teamwork attitude.
  • Interpersonal Skills: Maintain confidentiality, remain open to others' ideas, and demonstrate a willingness to try new approaches.
  • Technical Proficiency: Basic computer skills using Microsoft Office products and EHR systems.
