Acentra Health is looking for a Service Support Facilitator to join our growing team.

Job Summary:

• The purpose of this position is to facilitate the delivery of service support and administrative functions necessary for effective operation and delivery of services.

Responsibilities:

• Schedule, conduct and appropriately document member assessments within prescribed timelines.
• Enter assessment data into the web system.
• Participate in inter-rater activities to ensure consistency in the administration of the assessment instruments and adherence to assessment protocols.
• Participate in monthly staff meetings, quarterly all-staff meetings and other required training and meetings as necessary.
• Support in overseeing department procedures and participating in quality assurance/improvement initiatives.
• Conduct educational sessions for family members and agencies, covering policies related to assessments and budgetary matters.
• Complete satisfaction survey with a focus on achieving positive results from respondent feedback.

Required Qualifications:

• Bachelor's Degree in Human Services (Social Work, Criminal Justice, or psychology)
• Requires a minimum of 2 years of experience with the children's mental health
• Must Live in the State of West Virginia

Preferred Qualifications:

• Experience in a healthcare setting preferred
• Mastered Degree Preferred
• Medicaid/Public Sector service experience is also preferred.
