Description

General Summary:

The Housing Case Manager for Housing Problem Solving is responsible for implementing Homeless Prevention, Diversion and Rapid Exit strategies in order to assist individuals experiencing homelessness or housing insecurity in accessing pathways to stable housing.

Essential Responsibilities:




  • Provide homeless prevention strategies through housing related counseling, landlord mediation, budget education, connection to legal services and other supports for individuals facing homelessness or housing insecurity
  • Facilitate focused conversations aimed at diverting individuals from entering into the homeless system
  • Assist individuals who have already entered into the homeless system to identify and access Rapid Exit strategies to help them move as quickly as possible back into housing with the support of services and a minimal level of financial assistance
  • Identify and facilitate short term financial assistance for individuals who meet criteria for Housing Problem Solving funding
  • Assist clients in developing and implementing a housing plan to retain permanent housing as soon as possible after financial assistance ends
  • Collaborate with other service providers through case conferencing as needed in support of our common mission
  • Provide information and referrals to other providers
  • Ability to identify social determinants of health and coordinate appropriate referrals as needed (i.e. state and local benefits such as Medicaid, Supplemental Nutrition Assistance Program (SNAP), disability benefits, veterans' benefits, etc.
  • Monitor and evaluate program participant progress by reporting all client-level data to HMIS (Homeless Management Information System) on a daily basis
  • Coordinate with HMIS Agency Manager to ensure all training, updates and policies are followed
  • Maintain client and activity records related to all Housing Problem Solving activities
  • Adhere to Housing Problem Solving program standards
  • Maintain timely, accurate records, documentation, and reports as required
  • Accept other duties and responsibilities as assigned



Additional Responsibilities:




  • Attend all agency/department required training and meetings within the prescribed period.
  • Attend all Safety Training programs and be able to describe his/her responsibilities related to the general safety, department safety, and specific job-related hazards and responsibilities.
  • Demonstrate respect and regard for the dignity of all clients, families, visitors and fellow employees to ensure a professional, responsible and courteous environment.

  • Present a neat appearance in proper attire and identification as required by the position, department and agency policies and procedures.

Education, Experience and Competencies:



  • Associates Degree or equivalent combination of education and experience from a two-year college or technical school and three years related experience.
  • Previous experience supporting families or individuals with special care needs or disabilities or families or individuals accessing housing programs and services
  • Demonstrated ability to work both independently and as an effective team member
  • Demonstrated experience working with diverse populations
  • A combination of education and experience demonstrating acquisition of the skills and abilities required



Certifications, Licenses Requirements:




  • Valid driver's license and automobile insurance which meets Rhode Island minimum standards required, or the ability to obtain necessary transportation in order to perform the responsibilities/tasks of the job.
