Job Summary and Responsibilities

Acentra Health is looking for a Clinical Care Coordinator Supervisor (RN) to join our growing team.

Job Summary:

The Clinical Care Coordinator Supervisor (RN) role will oversee and manage the Care Coordination activities. One of their primary focus is to maintain high standards in clinical programs and enhance customer relationships to ensure contract requirements are met successfully.

*This position is hybrid within the State of Arkansas as there may be some travel to support, supervise, and train along with team meetings, and beneficiary visits. Location preference is the Central region of Arkansas but not required.

Responsibilities:

  • Manage and oversee Care Coordination activities, such as development and implementation of Person-Centered Service Plans (PCSP)
  • Ensure overall consistency, thoroughness, and accuracy of clinical programs
  • Oversee program to ensure clinical efficiencies, deliverables, and contract functions meet high standards
  • Strengthen relationships with customers and meet contract requirements
  • Provide day-to-day oversight to manage the care coordination program
  • Responsible for care coordination activities as defined by the contract, to include mentoring team members in care coordination best practices and performing quality audits on team member work products
  • Serve as a Subject Matter Expert (SME)
  • Participate as a team member fostering collaborative decision-making among leadership, committees, teams, or work groups of diverse composition
  • Some travel to support/supervise/train team conducting treatment team meetings, and beneficiary visits
  • Perform other duties as assigned

Qualifications

Required Qualifications

  • Active, unrestricted licensed Registered Nurse (RN) in Arkansas or compact license
  • Associate degree or equivalent directly applicable experience in nursing, health science or related area (bachelor's degree preferred)
  • Minimum of 4 years current experience in care coordination (field environment preferred)
  • Minimum 1 year of recent experience in a clinical environment working with individuals with developmental or intellectual disabilities or in behavioral health setting; preferably in field delivery
  • Minimum of 1 year of supervisory experience
  • Knowledge and familiarity with public sector health coverage systems, e.g. Medicare / Medicaid
  • Knowledge of principles, ethics, and precepts of care management service delivery
  • Experience in developing and reviewing care plans/plans of care
  • Experience facilitating interdisciplinary treatment team meetings

Preferred Qualifications

  • Excellent verbal and written communication skills essential
  • Customer-focused, results-oriented, and capable of building and maintaining relationships with internal and external customers
  • Organizational skills, ability to plan and prioritize multiple assignments essential
  • Ability to work in team setting and independently
  • Computer proficiency in care coordination software, Microsoft Office applications and other software programs essential to perform job functions
  • Ability to provide and receive verbal and written feedback in a professional manner and implement performance and productivity improvements as needed
  • Ability to provide technical guidance and leadership to management team

Pay Range

USD $62,400.00 - USD $79,040.00 /Yr.
