Job Details Job Description 003707PRIMARY FUNCTION:Under the general direction of the Population Health Management team the Program Coordinator participates and delivers services in a variety of Population Health initiatives. This position will provide assistance to both patient and health care teams for identification, eligibility and enrollment in programs as well as the monitoring and collection of outcomes data related to the various programs administered.ESSENTIAL FUNCTIONS:* Accepts referrals and manages assigned caseload for the identification and connection to community resources.* Determines insurance eligibility and assists in enrollment and any issues post enrollment.* Provide enrollees with information about the health insurance exchange and the health plans that are available.* Assists the care team in determining whether a patient qualifies for subsidies or Medicaid, and assists with the enrollment process.* Provides outreach and education about health-related social needs (insurance navigation, food insecurity, transportation)* Assists with the coordination and tracking of the various Population Health Initiatives* Performs audits/ reconciliation to ensure program targets and goals are met; running monthly Ad Hoc reports upon request; and notifying Population Health Leadership of identified process problems and/or audit outcomes.* Performs other duties as assigned.OTHER FUNCTIONS AND COMPETENCIES needed in addition to Essential Functions:(List Functions here.):* Excellent written and verbal communication skills* Proficiency in Microsoft suite and ability to multi-task on various digital platforms* Experience in data collection and entry* Penny Certification within 90 days of hire* Compass PA Navigation* GetCoveredNJ Certified within 90 days of hire* NJ FamilyCare Navigation* Motivational Interviewing* Maintain accurate and up-to-date knowledge of community resources and referral processesINTERNAL/EXTERNAL CONTACTS:Population Health, Center for Community Connectiveness, JCHE_________________________________________________________________________EDUCATIONAL/TRAINING REQUIREMENTS:High School Diploma,CERTIFICATES, LICENSES, AND REGISTRATION: _________________________________________________________________________EXPERIENCE REQUIREMENTS:__________________________________________________________________ADDITIONAL INFORMATION:* Strong problem-solving and organizational skills* Ability to work independently and as part of a team Work Shift Rotating (United States of America) Worker Sub Type Regular Primary Location Address 1101 Market, Philadelphia, Pennsylvania, United States of America Nationally ranked, Jefferson, which is principally located in the greater Philadelphia region, Lehigh Valley and Northeastern Pennsylvania and southern New Jersey, is reimagining health care and higher education to create unparalleled value.
