Nurse Practitioner or Physician Assistant, Fully Paneled - Delafield Primary Care - 1.0 FTE

We are seeking a highly skilled and experienced Nurse Practitioner (NP) or Physician Assistant (PA) to join our Delafield Clinic as a fully-paneled provider, with your own patient panel and full practice authority. In this role, you will be a critical member of our primary care team, delivering comprehensive care to patients across the lifespan. This position is ideal for an advanced practice provider with a strong foundation in family medicine or primary care who is ready to manage a full panel of patients independently.

The Delafield clinic is one of PHMG's 15 clinic locations in Waukesha County, offering a personalized care experience for both adult and pediatric patients. With a focus on Internal Medicine and Pediatrics, our clinic is known for its patient-centered approach, fostering long-term relationships with families and individuals.
