Care Coordinator/Clinician I - Skagit MCOT Work 4- 10's SUMMARY The Clinician I provide direct, supportive, and brief coordination of crisis inventions, and assist clients with achieving outpatient services when necessary to achieve the highest level of independence and stability possible in their community. Services delivered are recovery-focused and include but are not limited to case management, crisis invention & treatment planning, resource identification and assistance, identification and development of natural supports, advocacy, service monitoring and follow-up, collaboration with natural supports and other providers, and individual and group services. The Clinician I is responsible for assessing client functioning and needs and partnering with the client to develop an individually tailored care plan designed to promote recovery. The Clinician I also serve as a crisis team member and provides back up to other team members as needed. ESSENTIAL DUTIES AND RESPONSIBILITIES include the following, other duties may be assigned
- Behaves in an ethical and professional manner consistent with agency policy, state law, and licensing entities if applicable.
- Complies with all accurate and appropriate charting, coding, and billing procedures as required by the regulatory organizations.
- Document's delivery of services in the medical record. Completes all paperwork to agency standard and within required timelines.
- Meets or exceeds service hour expectations established by the agency.
- Accepts primary responsibility for the brief inventions with crisis clients as identified
- Establishes a treatment plan and goals with client and evaluates progress throughout brief crisis inventions.
- Routinely assesses for risk issues including danger to self or others and develops plans with client to mitigate those risks.
- Provides a range of medically necessary clinical interventions, including but not limited to behavioral health counseling and case management services, to adults/older adults/youth. Whenever possible, treatment approaches employing Evidence-Based Practices will be used.
- Provides group patient education including planning, implementing, and facilitating groups for identified clients.
- Works collaboratively and coordinates with psychiatric services, emergency services, outpatient services, other treatment team members, etc.
- Coordinates services with community agencies (e.g., DSHS, DDD, Courts, etc.) and works in context of a crisis team as needed and appropriate.
- Makes appropriate referrals for auxiliary services such as drug and alcohol services when clinically indicated.
- Coordinates care with primary care and collaborating professionals from other systems as well as natural supports as appropriate.
- Participates in regularly scheduled supervision and team meetings.
- Remains compliant with required trainings and certifications.
- Understands and practices safekeeping of client protected health information per Compass Health's HIPAA and other agency policies and procedures.

PROGRAM SPECIFIC DUTIES AND RESPONSIBILITIES
- Provides crisis invention & prevention services to persons with severe and persistent behavioral health disorders with complex needs, diverse diagnoses, and multi-system involvement.
- Provides outreach to client homes and facilities relevant to client care including hospitals, jails, medical facilities, and community organizations.
- Provides resource outreach support to client (s), and crisis team members.
EDUCATION / EXPERIENCE / CERTIFICATIONS / LICENSES
- BA/BS/BSW Degree in a Behavioral Science related field.
- Demonstrated knowledge and experience with Evidence-Based Practices, or interest and ability to learn.
- Individuals who do not already hold a license or associate license through the Washington State Department of Health must submit Agency Affiliated Counselor application upon hire and be approved by the DOH.
- Valid Washington State Driver's License including vehicle and appropriate insurance, if applicable.

- Excellent communication skills, written and verbal.
- General computer proficiency, including the ability to utilize electronic medical record systems.
- Ability to pass pre-employment criminal background check, which may or may not include an additional search by DSHS.
PHYSICAL DEMANDS / WORK ENVIRONMENT The employee will be working in a clinical office environment and will provide outreach to community settings as needed, which could include client homes, hospitals, medical facilities, schools, jails, community organizations, etc. While performing the duties of this job, the employee must occasionally lift and/or move up to 20 pounds and is regularly required to sit, stand, walk, talk, and use close vision. The noise level in the work environment is usually moderate.
