Pharmacist Ambulatory Care Full Time Days

Job Description:

The Ambulatory Care Pharmacist is responsible for providing comprehensive clinical pharmacy services for ambulatory patients through a wide variety of actions including: accurate medication reconciliation, medication review/disease state management, patient education (includes adherence education/assessment), and healthcare professional education.

Principal Duties and Responsibilities

The following is a summary of the major functions of this individual's job. He/she may perform other duties, both major and minor, not mentioned below, and specific functions may change from time to time.

  • Partners with providers and clinical staff to manage and coordinate patient directed pharmaceutical care.

  • Communicates with physicians and other clinicians as necessary to provide clinical pharmacy services for new and existing clinic patients.

  • Maintains onsite clinic pharmacy services, operational policies and procedures, clinical pharmacy workflows and practice expectations.

  • Participates actively in multidisciplinary committees, teams, working groups, and/or task forces related to the care of patients as the pharmacy representative.

  • Ensures all state and federal regulatory requirements and accreditation standards are met and appropriately documented.

  • Refers appropriate patients to financial counseling for copay assistance through manufacturer or charity programs.

  • Participates in the PGY1 and PGY2 residency programs, precepting students and residents when applicable

Minimum Education, Training, and Experience Required

  • Kentucky and/or Indiana Board of Pharmacy license in good standing.

  • Bachelor's Degree in Pharmacy or Doctorate of Pharmacy required.

  • Minimum of 3 years related work experience in ambulatory practice, completion of ASHP accredited pharmacy residency program, or Board Certification in relevant practice area required.

  • BLS certification required

  • Immunization certification preferred
